The fourth-degree tear, also known as fourth-degree laceration, is the most serious form of a vaginal tear. It extensively goes through the perineum and vaginal tissue and even stretches into the rectum and anal canal. Though this type is severe, it is also the least common form. Women at the first vaginal delivery tend to be at a higher risk. Also, it is more common in people of Asian descents or those who go through an episiotomy.  [2]
